Job summary

WelbeHealth, LLC. is seeking Home Care Assistants to provide in-home personal care and support to participants in Carson, CA, as part of our interdisciplinary team.

You will follow care plans, document vital signs, and assist with daily living activities in a patient-centered, value-based program. Qualifications

• High school diploma or equivalency is required.
• Current BLS/First Aid certification is required.
• CNA or HHA certification preferred.
• Must be registered with Home Care Registry prior to start date.

Responsibilities

• Provide hands-on personal care and assistance with ADLs in participants' homes (e.g., bathing, dressing, grooming, toileting, transferring).
• Assist with IADLs (laundry, light housekeeping, shopping, light meal prep).
• Measure and document vital signs as instructed.
• Act as medical escort to appointments and assist with safe transfers.
• Carry out home exercise programs as indicated in the care plan by the IDT.
