Job summary

WelbeHealth is hiring Home Care Assistants to deliver in-home personal care and support to participants in their homes as part of a multidisciplinary team. You will contribute to care planning and ensure compassionate, hands-on support for daily activities.

The role offers a stable full-time schedule, travel between participants, and a comprehensive benefits package, with compensation based on experience and qualifications. Bilingual candidates are encouraged to apply.

Qualifications

• Current BLS/First Aid Certification required.
• Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred.
• 1+ years of caregiving experience with elderly or senior patients highly preferred.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living (ADLs) in participants' homes, including bathing, dressing, grooming, toileting, and safe transfers.
• Assist participants with Instrumental ADLs such as laundry, light housekeeping, shopping, and meal preparation.
• Measure and document participants’ vital signs as instructed.
• Act as a medical escort on vehicles to medical appointments and assist with safe transfers.
• Support Rehabilitation team by carrying out home exercise programs as indicated in the care plan.
• Provide translation services as needed.
