Job summary

WelbeHealth, LLC. is seeking Home Care Assistants to deliver personal care and support to participants in their homes, working as part of an interdisciplinary team. You will help with daily living activities, mobility, and participating in care plans.

Qualifications

• High school diploma or equivalency is required.
• Current BLS and First Aid certifications are required.
• CNA or HHA certification is preferred but not mandatory.
• Must be registered with the Home Care Registry before start date.
• CPR and First Aid certification within 180 days of hire is preferred and must be maintained.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living (ADLs) in participants' homes.
• Assist with Instrumental ADLs such as laundry, light housekeeping, shopping, and light meal preparation.
• Measure and document participants’ vital signs as instructed.
• Act as a medical escort to appointments and assist with safe transfers.
• Carry out home exercise programs as indicated in the care plan developed by the IDT.
• Provide translation services as needed.
