Job summary

WelbeHealth is seeking a Home Care Assistant to provide safe personal care and participate in care planning for participants in their homes, reporting to the Home Care Supervisor as part of the IDT.

You will visit 2–5 patients daily, assist with daily activities, document vitals, and help with home exercise programs. WelbeHealth offers comprehensive benefits and competitive compensation.

Qualifications

• High school diploma or equivalency.
• Current BLS/First Aid Certification required.
• Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred.
• Must be registered with Home Care Registry prior to start date.
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living (ADLs) in participants' homes.
• Assist with instrumental ADLs such as laundry, light housekeeping, shopping, and meal prep.
• Measure and document vital signs as instructed.
• Act as a medical escort to appointments and help with safe transfers.
• Support Rehabilitation team by carrying out home exercise programs as needed.
