Affinity Hospice is seeking a Registered Nurse/Case Manager to plan, organize, direct, and provide hospice care in home and community settings. The role emphasizes clinical leadership, education, and coordination with families and interdisciplinary teams.

The position requires an RN license, CPR certification, and at least two years of RN experience in home health or hospice. The schedule is full-time with on-call duties, and the organization offers comprehensive benefits and opportunities for

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse.
• Current CPR certification.
• Minimum of two years of RN experience in clinical care, home health or hospice preferred.
• Must be a licensed driver with insured vehicle in good working order.
• Excellent observation, verbal and written communication skills, problem solving, basic math; nursing skills per competency checklist.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care.
• Provide care utilizing infection control measures that protect staff and patient (OSHA).
• Assure continuity of quality patient care with proper documentation.
• Monitor assigned cases for third-party payer requirements.
• Demonstrate commitment, professional growth and competency.
• Promote agency philosophy and administrative policies.
• Perform on-call responsibilities and provide on-call service to patients/families.
