Job summary

Affinity Hospice is seeking a Registered Nurse/Case Manager to plan, organize and direct hospice care in patients’ homes and communities. The role emphasizes community health education and coordinating an interdisciplinary team to meet patient and family needs.

The position is full-time (40 hours/week) with responsibilities including case management, care planning, and on-call coverage. A nursing degree, active RN license, CPR certification, and driving eligibility are required.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in the state where practicing.
• Current CPR certification.
• Minimum of two years of RN experience in clinical care, home health or hospice preferred.
• Must have a valid driver’s license and insured vehicle.
• Excellent verbal and written communication; good math and problem-solving skills.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement, develop and document the patient plan of care.
• Provide care maintaining infection control and OSHA standards.
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases for compliance with third-party payer requirements.
• Demonstrate professional growth and adherence to agency policies.
• Perform on-call duties for patients and families as assigned.
• Supervise hospice aides and licensed practical nurses.
