Job summary

Affinity Hospice is seeking a registered nurse/case manager to plan, coordinate, and provide hospice care in patients' homes and communities. The role emphasizes community health education and direct patient engagement by a skilled RN.

The position is full-time (40 hours/week) with responsibilities that include managing the patient plan of care, supervising hospice aides, and ensuring compliant documentation and care quality.

Qualifications

• Fulfills RN licensure requirements and home health or hospice experience preferred.
• Excellent observation and communication skills and problem solving abilities.
• Basic math competency and nursing skills per competency checklist.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement and document patient plan of care.
• Provide care with OSHA infection control measures.
• Ensure continuity and quality of patient care with proper documentation.
• Monitor cases for third-party payer requirements.
• Supervise hospice aides and licensed practical nurses.
• Perform other nursing duties as assigned by management.
