Job summary

Reserve Care, formerly Hospice of the Western Reserve, is seeking a Home Care Registered Nurse (RN) for full-time day shifts in Willoughby Hills, OH. You will deliver prescribed medical treatment and personal care in collaboration with the care team, following the Plan of Care and RC standards.

Requirements include an active Ohio RN license, CPR certification, and a valid driver’s license. Hospice/palliative experience is preferred; on-call and travel may be required.

Qualifications

• Licensed RN in State of Ohio.
• Current CPR certification.
• Must have and maintain a valid driver’s license.

Responsibilities

• Provide prescribed medical treatment and personal care services to terminally ill patients.
• Coordinate the Plan of Care with the transdisciplinary team.
• Participate in care conferences and discharge planning.
• Maintain complete plans and reports related to hospice care.
