Job summary

Hospice of the Western Reserve, now Reserve Care, is hiring a Registered Nurse (RN) for full-time day shift in Medina County, Medina, OH. The role involves providing prescribed medical treatment and personal care to terminally ill patients in collaboration with a transdisciplinary team.

You will assess needs, administer skilled nursing services, monitor pain and symptoms, educate patients and families, and participate in care conferences and quality improvement.

Qualifications

• Licensed RN in the state of Ohio.
• Current CPR certification.
• Must have and maintain a valid driver’s license and good driving record.

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients.
• Coordinate Plan of Care with the transdisciplinary team.
• Assess patient, family, and caregiver needs (physical, psychosocial, spiritual).
• Deliver intermittent skilled nursing services under physician direction.
• Monitor pain and symptoms; educate patients and families on management.
• Serve as liaison to physicians and document patient needs.
• Provide emotional support to patients and families.
• Refer to other services when appropriate; follow up as needed.
• Provide health teaching to patients and families.
• Complete plans and reports related to hospice care.
• Participate in care conferences and bereavement follow-up.
• Participate in quality improvement activities and committees.
• Supervise activities of healthcare aides as related to patient care.
• Participate in on-call rotations and discharge planning.
