Job summary

Reserve Care, formerly Hospice of the Western Reserve, is seeking a Home Care Registered Nurse (RN) for full-time day shifts in Fairview Park, OH. You will deliver prescribed medical treatments and personal care to terminally ill patients in collaboration with a transdisciplinary team at diverse settings.

You will assess needs, manage pain, educate families, and coordinate the Plan of Care with physicians. A current OH RN license, CPR, and valid driver’s license are required; hospice experience

Qualifications

• Licensed RN in Ohio.
• Hospice/home health experience preferred (oncology welcome).
• Current CPR certification.
• Valid driver’s license with clean driving record.
• Hospice/Palliative Care certification desired; within 2 years of hire.

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients.
• Coordinate the Plan of Care with the transdisciplinary team.
• Assess patient and family needs; manage pain and symptoms.
• Serve as liaison to physicians and communicate patient needs.
• Provide patient and family education and emotional support.
• Maintain patient records and complete reports and plans.
