Job summary

Hospice of the Western Reserve (Reserve Care) is seeking a Home Care Registered Nurse (RN) for full-time day shifts in Fairview Park, OH. You will join a multidisciplinary team to provide skilled nursing and personal care to terminally ill patients at home.

The role requires Ohio RN licensure, CPR certification, and a valid driver’s license, with responsibilities including Plan of Care implementation and family education.

Qualifications

• Current Ohio RN license
• Experience in hospice/home health/oncology and pain management preferred
• Current CPR certification
• Valid driver’s license with reliable auto insurance
• Ability to work in various settings with flexible hours

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients in collaboration with the transdisciplinary team.
• Coordinate the implementation of the Plan of Care for each patient utilizing the Nursing Process.
• Assess physical, psychosocial, and spiritual needs of the patient and family/caregivers.
• Provide intermittent skilled nursing services under physician direction.
• Monitor and manage pain and symptoms; educate patient and family.
• Serve as liaison to physicians and coordinate with other services as needed.
• Participate in care conferences, discharge planning and on-call rotations.
