Reserve Care is hiring a Registered Nurse (RN) for full-time day shift in Medina, OH. Qualifications

• Active Ohio RN license.
• Experience in hospice or palliative care preferred.
• Current CPR certification.
• Valid driver's license required.

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients with the multidisciplinary team.
• Coordinate and implement the Plan of Care using the Nursing Process.
• Assess physical, emotional, and spiritual needs of patients and families.
• Monitor pain and symptoms; educate patients and families on symptom management.
• Liaise with physicians; communicate patient needs and information.
• Participate in on-call rotation and discharge planning as needed.
• Maintain records, reports and competency requirements.
• Support volunteers and coordinate care with the care team.
