Job summary

Hospice of the Western Reserve (Reserve Care) seeks a Home Care Registered Nurse (RN) for full-time daytime hours in the Fairview Park, OH area. You will provide prescribed medical treatment and personal care to terminally ill patients, working with a transdisciplinary team to implement the Plan of Care.

You will assess physical, psychosocial, and spiritual needs, monitor pain and symptoms, and educate patients and families while coordinating care and participating in on-call rotations and

Qualifications

• Licensed RN in the State of Ohio.
• Current CPR certification.
• Must have and maintain a valid driver’s license and auto insurance as a licensed driver in OH.

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients.
• Coordinate Plan of Care with the transdisciplinary team.
• Assess physical, psychosocial, and spiritual needs of patients and families.
• Provide intermittent skilled nursing services under physician direction.
• Monitor pain and symptoms and educate patients and families on management.
