Responsibilities

• Provide companionship and conversation to enhance client lives.

• Accompany clients to appointments or social outings and provide transportation as needed.
• Monitor and report changes in health, behavior, or needs to RN Case Manager.
• Collaborate with Care coordination team to update the care plan.

We are currently seeking a dedicated and adaptable Home Care Field Specialist to join our team. This full-time role requires providing high-quality home care services to clients with diverse and often complex needs. The ideal candidate must be comfortable working with Hoyer lifts and have reliable transportation, valid drivers license and current auto insurance. This position offers 30 guaranteed hours per week, with a required 40-hour weekly availability to ensure flexible scheduling and coverage. Must be comfortable traveling within a 35 mile radius of Duluth as we service clients in Duluth and the surrounding cities.

• Premium Short Shift Pay: We offer a variety of shift Lengths ranging from 1 hour to 12 hours. Our 1-hour shifts get paid at $30 per hour and hour 2-hour shifts get paid at $22.50 an hour. Any shift 3 hours or longer is paid at a base rate between $19-$22 based on experience.

Role Responsibilities:

• Provide physical assistance such as bathing, dressing, grooming, transferring, and ambulation.


• Accompany clients to appointments or social outings, providing transportation as needed.


• Monitor and report changes in the client’s health, behavior, and needs.


• Act as a member of the Care coordination team to communication with the RN Case Manager to update the plan of care.

Requirements:

• A compassionate, patient, and caring nature.


• Strong interpersonal and communication skills.


• Experience providing care to clients in a hands-on environment (Home Health, Assisted Living, Group Home, Hospital Setting)


• Reliable vehicle and a valid driver’s license with Auto Insurance.


• Negative TB test or Cleared Chest-Xray.


• Availability to work 2 weekend days per month.
