Terros Health in Phoenix, AZ is hiring a Full-Time Case Manager at the 27th Ave Health Center. You will work as part of an interdisciplinary team to support individuals with mental health needs, helping them improve daily functioning, independence, and quality of life.

The role emphasizes patient-centered, strength-based approaches, with service coordination, skill-building, and connection to housing, employment, and community resources.

Responsibilities

• Provide direct, face-to-face services in the home, community, or clinic that support treatment goals.
• Teach and coach skills related to daily living, health, and recovery.
• Engage individuals using strength-based, trauma-informed, and recovery-oriented approaches.
• Proactively re-engages individuals who are disengaged, difficult to reach, or at higher risk.
• Engages regularly with individuals to identify and implement goals and interventions.
• Support individuals in building skills related to independent living, housing stability, employment, relationships, and self-care.
• Help individuals navigate real-life challenges and practice skills in real-world settings.
• Develops and implements individualized service plans focused on functional outcomes.
