Terros Health is seeking a Case Manager to join our interdisciplinary clinical team at the Desert Cove Recovery Health Center in Phoenix, AZ. • Assist clients with identifying treatment and recovery goals and provide supportive services.
• Complete assessments, crisis plans and individualized service plans.
• Collaborate with clinical team to engage, educate, communicate and coordinate care with members, families and providers.
• Conduct outreach and follow-up to ensure resources and services are available.
• Perform caseload management including assessments and service planning processes.
