Job summary

Elevate Mental Wellness in Pueblo is seeking a Care Coordinator to be the first contact for patients, ensuring a welcoming, organized, and supportive experience. This onsite role supports patients starting care while offering remote administrative help to other clinic locations.

The position focuses on scheduling, phone and message management, and coordination of care-related tasks. It is a full-time, on-site role with a 4×10-hour Tue–Fri schedule and benefits including 401(k), health and vision

• Experience in mental health care preferred.
• Demonstrates professionalism, empathy, and composure in high-stress situations.
• Highly organized, dependable, and attentive to detail.

Responsibilities

• Answer and route phone calls; respond to patient inquiries professionally and compassionately.
• Schedule appointments and manage clinic calendars
• Greet patients and maintain smooth front desk operations
• Assist clinical providers with prior authorizations, referrals, vital signs, and drug testing as needed
• Track tasks, messages, and follow-ups with accuracy and accountability
