• Home care, hospice, senior living or healthcare referral experience is a strong plus, not required.

Responsibilities

• Own the private-pay pipeline from source visits to start of care.
• Build referral relationships with hospitals and social workers.
• Lead and recruit liaisons once target is met.
• Track KPI dashboards and report to owner.
