Job summary

Health Connect America is seeking a Foster Care Case Manager to support children and families in the foster care system, coordinating medical, educational, and behavioral health services to ensure coordinated care. You will maintain documentation and serve as a key liaison among foster parents, biological families, providers, and internal teams.

The role emphasizes relationship-driven, detail-oriented work with a focus on long-term outcomes and compliance with COA and state standards.

Qualifications

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
• Experience must be full-time and supervised, serving children and adolescents with a primary mental health diagnosis.

Responsibilities

• Develop and implement individualized treatment and service plans.
• Coordinate medical, educational, behavioral health, and social services for seamless care.
• Serve as a central point of communication between foster parents, families, providers and internal teams.
• Monitor progress and adjust care plans to meet evolving needs.
• Maintain accurate case records per standards.
• Partner with foster parents to provide guidance and resources for safe homes.
• Collaborate with internal teams and external agencies to ensure coordinated service delivery.
