First Home Care, supported by Health Connect America, is seeking a Foster Care Case Manager to provide compassionate, relationship-driven services for children and families in Virginia's foster care system.

Responsibilities include coordinating services, developing individualized plans, and building strong partnerships with foster parents and treatment teams to achieve permanency and family stability.

Formación

• MSW or PhD in social work with supervised field placement or caseload experience
• Bachelor's in social work or related field plus one year of casework with children/families
• Bachelor's in any field plus two years of casework experience with children and families

Responsabilidades

• Develop and implement individualized service plans for children in foster care and monitor progress
• Coordinate medical, educational, behavioral health, and community services for families
• Build relationships with foster parents, birth families, and partners to support permanency goals
• Maintain accurate case records and ensure compliance with Health Connect America and standards
• Support foster parent recruitment, assessment, and training, and provide crisis guidance as needed
