Job summary

FamilyForward, a nonprofit focused on supporting vulnerable children through therapeutic and education services, is seeking a Foster Care Case Manager. You will decide on client service plans across medical, educational, social, and emotional needs, ensuring access to necessary resources for quality care.

You will handle on-call emergencies, coordinate permanency goals, prepare detailed records, and lead Family Support Team meetings.

Qualifications

• Bachelor's degree required.
• Experience in foster care and/or with at-risk youth and families preferred for case management roles.
• Strong teamwork and collaboration to achieve best results for children and families.
• Ability to manage multiple demands, make independent decisions, and use resources efficiently.
• Commitment to innovative approaches to positive outcomes for children and families.

Responsibilities

• Make decisions related to client services for medical, education, social, and emotional supports and ensure access to identified resources.
• Maintain on-call coverage to provide emergency support to child, family, and state agency.
• Formulate permanency goals and plans for assigned clients.
• Prepare records and documentation including assessments, treatment plans, case plans, evaluations, and termination summaries.
• Lead and attend Family Support Team meetings and coordinate with biological parents, children, and caregivers.
• Attend court hearings when required by supervisor.
• Transport clients to appointments, court, school, and other events.
• Identify and coordinate links to community resources for clients.
