Job summary

Health Connect America is seeking a Foster Care Case Manager to oversee service planning, counseling, supervision, and case management for children and families in the foster care system. You will develop individualized treatment plans and coordinate medical, social, educational, and related services to meet each child’s needs.

The role requires a Bachelor’s degree in social work or a related field and at least two years of relevant experience, with state-specific requirements for North Carolina

Qualifications

• Bachelor’s degree in social work or a related human service field is required for this position.
• A minimum of two (2) years of relevant experience is required to qualify for this role.

Responsibilities

• Develop and implement individualized treatment plans for children in foster care.
• Prepare and maintain detailed case records and weekly activity/status reports.
• Coordinate and link children to services to minimize fragmentation and ensure access to needed services.
• Engage with foster parents to support family success and child welfare.
• Collaborate with other agencies to arrange medical, remedial, and dental services as needed.

Education

Bachelor’s degree in social work or a related human service field

A minimum of two (2) years of relevant experience
