Job summary

Health Connect America, Inc. seeks a Foster Care Case Manager to support children and families within the foster care system across North Carolina. You will coordinate services, develop individualized plans, and ensure safety and well‑being in a compassionate, trauma‑informed framework.

The role requires NC QP credentials or an equivalent license, plus experience with youth, supervision, and a commitment to high‑quality, compliant care within a supportive, mission‑driven team.

Qualifications

• Must meet NC Qualified Professional (QP) requirements.
• Master’s in human services field + 1 year experience.
• Bachelor’s in human services field + 2 years experience.
• Bachelor’s in non‑human services field + 4 years experience.
• Current NC license or board‑issued certification in a human services profession.

Responsibilities

• Develop and implement individualized treatment and service plans to support each child’s safety, stability, and well‑being.
• Coordinate medical, educational, behavioral health, and social services to ensure seamless, uninterrupted care.
• Serve as a central point of communication between foster parents, biological families, service providers, and internal teams.
• Facilitate and support healthy interactions between children, foster families, and birth families to promote positive outcomes.
• Monitor and continuously assess each child’s progress, adjusting care plans to meet evolving needs.
• Maintain accurate, timely documentation and case records in accordance with organizational and regulatory standards.
• Partner with foster parents to provide guidance, support, and resources for safe, nurturing homes.
• Collaborate with internal teams and external agencies to ensure coordinated service delivery and compliance with all requirements.
