Job summary

Health Connect America, Inc. is seeking a Foster Care Case Manager to oversee service planning, counseling, supervision, and case management for children and families in foster care.

You will develop individualized treatment plans, coordinate services, and ensure access to medical, social, educational, and other supports. The role involves collaboration with foster parents, birth families, and agencies, maintaining records, and ensuring compliance with COA and state requirements.

Qualifications

• Bachelor’s degree in social work or related human service field is required.
• Minimum of two years of relevant experience required.
• State-specific requirements vary by North Carolina and Virginia depending on degree and experience in child mental health services.

Responsibilities

• Develop and implement individualized treatment plans for children in foster care.
• Prepare and maintain detailed case records and weekly activity/status reports.
• Coordinate and link children to appropriate services to minimize care fragmentation and ensure access to services.
• Facilitate relationships between birth parents, foster families, and children to support family success.
• Monitor and adjust care plans to meet evolving needs.
• Recruit, assess, and train foster parents as needed.
• Coordinate services with other agencies involved with the child.

Education

Bachelor’s degree in social work or related human service field
