Job summary

Family Services of America is seeking a Medical Coordinator to arrange and coordinate health services for children in foster care, including urgent, emergent, and preventative care, and to maintain confidential medical records in the EHR.

You will communicate health needs across clinical and case management teams, ensure qualified interpretation, coordinate transportation, and act as a liaison with community providers. Grant-funded, term-limited for up to three years.

Qualifications

• Minimum 1 year in public health or healthcare with care coordination or health administration experience.
• Must have LPN/LVN or higher license OR a Bachelor's degree in a relevant field.
• Background checks and immunization proof required.

Responsibilities

• Coordinate urgent, emergent, and preventive health services, including scheduling and transportation.
• Maintain confidential medical records and document health information in the EHR.
• Ensure communication of health needs between medical, case management and clinical staff.
• Arrange qualified interpretation and translation services for appointments.
• Serve as liaison for community health providers and act as medical chaperone when needed.
• Track vaccination and immunization requirements for children in care.
