Responsibilities

• Participate in referral matching for children and families, advocating for best home match.
• Develop and facilitate each child's Initial Treatment Plan and Comprehensive Treatment Plan.
• Facilitate and ensure completion of Biopsychosocial, Independent Living, Diagnostic assessments as appropriate.
• Refer foster child to behavioral health, independent living, and other support services.
• Ensure safety plans and required medical/therapy services are provided.
• Provide training and support to foster parents and children; monitor progress and documentation.
