Job summary

The role emphasizes coordinating treatment plans, intake, assessments, and services, documenting activities, maintaining confidentiality, and staying compliant with state licensure while supporting families through training and ongoing casework.

Qualifications

• Minimum age 21 and valid driver’s license.
• LSW/LPC or related human services degree as required by state licensure.
• Experience with case management and working with children with behavioral needs is preferred.

Responsibilities

• Participate in referral matching and advocate for best homematch.
• Develop and monitor Individualized Treatment Plans and Comprehensive Treatment Plans.
• Facilitate completion of assessments (Biopsychosocial, Independent Living, Diagnostic).
• Refer foster child to behavioral health and other services.
• Ensure legal documentation and safety plans are completed.
• Coordinate with foster families and schools to meet education goals.
