Qualifications

• Active Minnesota Registered Nurse (RN) license.
• Previous experience in home health, hospital, acute care, rehabilitation, skilled nursing, or another clinical environment.
• Strong clinical assessment and critical-thinking skills.
• Confidence providing care independently in patients' homes.
• Strong organization and time-management abilities.
• Excellent patient, caregiver, and team communication.
• Flexible and adaptable approach to changing assignments.
• Ability to quickly become comfortable within new territories.
• Reliable transportation.
• Willingness to travel throughout our 21-county service area.
• Previous home health nursing experience is especially valuable given independence and adaptability required.

Responsibilities

• Provide skilled RN home health visits within your assigned coverage area.
• Rotate through different portions of our service area based on patient volume and staffing needs.
• Typically remain within each coverage territory for approximately three months.
• Complete comprehensive nursing assessments.
• Perform wound care, medication management, injections, and other skilled nursing services.
• Monitor patients with acute and chronic medical conditions.
• Identify and communicate changes in condition.
• Educate patients and caregivers on medications, diagnoses, treatments, and recovery.
• Coordinate care with physicians and healthcare providers.
• Collaborate with PT, OT, SLP, social work, and interdisciplinary team members.
• Help maintain continuity of care during changing census and staffing needs.
• Complete timely documentation in Homecare Homebase (HCHB).
