Qualifications

• Must hold a Minnesota RN license.
• Experience in home health, hospital, acute care, rehabilitation, or skilled nursing preferred.
• Ability to travel across a 21-county service area and adapt to rotations.

Responsibilities

• Provide skilled RN home health visits within your assigned coverage area.
• Rotate through different portions of the service area based on patient volume and staffing needs.
• Typically remain within each coverage territory for approximately three months.
• Perform wound care, medication management, injections, and other skilled nursing services.
• Monitor patients with acute and chronic medical conditions; identify changes in condition.
• Educate patients and caregivers on medications, diagnoses, treatments, and recovery.
• Coordinate care with physicians and healthcare providers; collaborate with interdisciplinary team members.
• Complete timely documentation in Homecare Homebase (HCHB).
