Job summary

Home Health Care, Inc. is seeking a Floating Visit Nurse (RN) in Minnesota to cover an expansive 21-county service area. You will rotate between territories every three months, delivering skilled home health visits, wound care, injections, and medication management while collaborating with multiple clinical teams.

You must hold an active Minnesota RN license and have strong clinical assessment and communication skills to provide high-quality, consistent patient care across diverse communities.

Qualifications

• Active Minnesota RN license required.
• Previous home health, hospital, acute care, rehab, or similar clinical experience preferred.
• Strong clinical assessment and critical-thinking skills required.
• Ability to independently provide care in patients' homes.
• Excellent organization and time-management abilities.
• Willingness to travel throughout the 21-county service area.

Responsibilities

• Provide RN home health visits within assigned coverage area.
• Rotate through different portions of the service area based on patient volume and staffing needs.
• Typically remain within each coverage territory for about three months.
• Perform wound care, medication management, injections, and other skilled nursing services.
• Monitor patients with acute and chronic conditions and communicate changes.
• Educate patients and caregivers on medications, diagnoses, and treatments.
• Coordinate care with physicians and other healthcare providers.
• Collaborate with interdisciplinary team members to ensure continuity of care.
• Complete timely documentation in the Homecare Homebase (HCHB) system.
