Job summary

Home Health Care, Inc. is seeking an experienced RN Case Manager for a float role across our Greater Twin Cities Metro territory. You will manage a home health caseload, rotate between territories, and perform Start of Care, Recertification, and Discharge assessments.

You’ll develop plans of care, provide skilled nursing visits, and coordinate with PT/OT/SLP and other team members. OASIS documentation and patient education are key responsibilities.

Qualifications

• Active Minnesota RN license required.
• Home health and OASIS experience preferred.
• Strong assessment and clinical decision-making skills required.

Responsibilities

• Manage an assigned home health caseload within a rotating territory.
• Rotate through service area based on clinical and staffing needs.
• Complete OASIS Start of Care, Recertification, and Discharge assessments.
• Develop and update individualized plans of care.
• Perform comprehensive nursing assessments and skilled visits.
• Provide wound care and medication management; patient education.
• Monitor progress and communicate with physicians and interdisciplinary teams.
• Coordinate with PT, OT, SLP, social work and others; ensure timely care.
• Maintain clinical documentation in Homecare Homebase (HCHB).
• Help prevent unnecessary hospitalizations; support continuity of care.
