Job summary

Home Health Care, Inc. seeks a Registered Nurse to join our Floating Visit Nurse team. You will provide skilled RN home health visits across an expanding 21-county service area, rotating between territories every few months.

The per-visit compensation model is based on volume, with an emphasis on adaptability and teamwork. You’ll learn routes, build patient relationships, and coordinate with clinicians to ensure continuous, high-quality care in patients’ homes.

Qualifications

• Active Minnesota RN license.
• Experience in home health, hospital, acute care, rehab, or another clinical environment.
• Strong clinical assessment and critical-thinking skills.
• Ability to travel throughout the 21-county service area.
• Excellent patient, caregiver, and team communication.

Responsibilities

• Provide skilled RN home health visits within your assigned coverage area.
• Rotate throughout different portions of our service area based on patient volume and staffing needs.
• Typically remain within each coverage territory for approximately three months.
• Perform wound care, medication management, injections, and other skilled nursing services.
• Monitor patients with acute and chronic medical conditions.
• Identify and communicate changes in condition.
• Educate patients and caregivers on medications, diagnoses, treatments, and recovery.
• Coordinate care with physicians and healthcare providers.
• Collaborate with PT, OT, SLP, social work, and interdisciplinary team members.
• Help maintain continuity of care during changing census and staffing needs.
• Complete timely documentation in Homecare Homebase (HCHB).
