Job summary

Home Health Care, Inc. in Northfield, MN, seeks an experienced RN Case Manager to provide clinical coverage as a Floating nurse across the 21-county Greater Twin Cities service area.

The role supports patient care continuity, with 3-month assignments, a Monday–Friday schedule, and opportunities for incentive visits, licensure reimbursement, and comprehensive benefits. You’ll assess patients, coordinate care with physicians and an interdisciplinary team, and ensure timely care in home settings as

Qualifications

• Active Minnesota RN license.
• Experience in home health and OASIS preferred.
• Ability to manage a patient caseload independently.
• Strong organizational and time-management skills.
• Excellent communication with patients, families, and physicians.

Responsibilities

• Manage a home health caseload across assigned territories.
• Complete OASIS Start of Care, Recertification, and Discharge assessments.
• Develop and update individualized plans of care.
• Perform wound care, medication management, and other skilled nursing services.
• Educate patients and caregivers on diagnoses, medications, and treatment plans.
• Monitor patients for changes in condition.
• Coordinate with physicians and interdisciplinary clinicians.
• Transition between territories as needs shift to maintain continuity of care.
• Maintain timely documentation using Homecare Homebase (HCHB).
