Home Health Care, Inc. (Minnesota) is seeking a Floating RN Case Manager to provide experienced clinical coverage across our 21-county Greater Twin Cities service area. You will step into territories with priority cases, own assigned caseloads, and maintain continuity of patient care.

The role emphasizes independence, adaptability, and collaboration with local teams. Assignments typically last about three months, with a focus on maintaining momentum in patient care while census shifts occur.

Qualifications

• Active Minnesota RN license.
• Strong clinical assessment and critical-thinking skills.
• Home health and OASIS experience preferred.
• Confidently manage a patient caseload.
• Strong organization and time-management abilities.
• Excellent communication with patients, caregivers, physicians and team.
• Ability to adapt as territory assignments change.
• Reliable transportation and willingness to travel across the service area.

Responsibilities

• Manage an assigned home health patient caseload.
• Complete OASIS Start of Care, Recertification, and Discharge assessments.
• Perform comprehensive nursing assessments and develop plans of care.
• Provide skilled nursing visits and wound care when needed.
• Educate patients and caregivers on diagnoses, medications, and treatment plans.
• Coordinate with interdisciplinary clinicians and physicians.
• Maintain timely documentation using Homecare Homebase (HCHB).
