Qualifications

• Active Minnesota RN license required.
• Experience in home health or clinical settings preferred.
• Ability to travel across a 21-county service area.
• Strong communication and organizational skills required.
• Ability to work independently in patients' homes.

Responsibilities

• Provide skilled RN home health visits within assigned coverage area.
• Rotate across service area based on patient volume and staffing needs.
• Typically remain within each coverage territory for approximately three months.
• Perform wound care, medication management, injections, and other skilled nursing services.
• Coordinate care with physicians and healthcare providers.
