Job summary

Home Health Care, Inc. is seeking a RN Case Manager for a flexible floating territory role in the Greater Twin Cities Metro Area. You will rotate across a 21-county service area, handling a caseload and performing OASIS assessments, care planning, and skilled visits.

The position is full-time, with an on-call rotation and a competitive compensation package. Earn $95,000–$130,000 annually with incentive visits and a $6,000 first-year bonus.

Qualifications

• Active Minnesota RN license in good standing.
• Home health and OASIS experience preferred.
• Ability to independently manage a patient caseload.

Responsibilities

• Manage an assigned home health caseload within your current territory.
• Complete OASIS Start of Care, Recertification, and Discharge assessments.
• Perform comprehensive nursing assessments and develop plans of care.
• Provide skilled nursing visits and wound/medication management.
