Job summary

Home Health Care, Inc. is seeking an experienced RN Case Manager for a Home Health Float role across Minnesota’s service area. You’ll rotate through territories, manage a home health caseload, and collaborate with multiple clinical teams to deliver high-quality patient care.

The position offers $95,000–$130,000 annually, incentive visit opportunities, and a $6,000 first-year bonus. Flexible rotations provide broad exposure and professional growth for a versatile nurse.

Qualifications

• Active Minnesota RN license required.
• Home health and OASIS experience preferred.
• Ability to manage an independent caseload.
• Strong organization and time-management skills.

Responsibilities

• Manage an assigned home health caseload.
• Rotate across service area based on clinical and staffing needs.
• Complete OASIS Start of Care, Recertification, and Discharge assessments.
• Develop and update individualized plans of care.
• Perform comprehensive nursing assessments and skilled visits.
• Coordinate with physicians, therapists, and the interdisciplinary team.
• Maintain timely and accurate clinical documentation in Homecare Homebase (HCHB).
