Job summary

Home Health Care, Inc. is seeking a Registered Nurse (RN) Case Manager for a Float role across its 21-county Minnesota service area.

The position offers a flexible territory rotation, typically three months per area, with opportunities to learn multiple communities and patient populations. You will lead and coordinate care, complete assessments, manage caseloads, and collaborate with multidisciplinary teams to enhance patient outcomes.

Qualifications

• Active Minnesota RN license or compact state RN license is required.
• Experience with OASIS assessments and home health care processes.
• Strong clinical assessment and decision-making abilities are essential.
• Ability to manage patient caseloads and coordinate multidisciplinary care.

Responsibilities

• Manage an assigned home health caseload within territory.
• Complete OASIS Start of Care, Recertification, and Discharge assessments.
• Develop and update individualized plans of care.
• Perform wound care, medication management, and other skilled nursing services.
• Educate patients and caregivers and monitor progress throughout episodes of care.
• Coordinate care with physicians and other health providers and interdisciplinary team members.
