Job summary

Qualifications

• Must be at least 18 years old.
• High School Diploma or GED or equivalent.
• Valid driver’s license in good standing.
• Must have reliable transportation.
• Must successfully complete all background checks.
• TB health screening may vary by state/program requirements.

Responsibilities

• Assist individuals with goals per individualized plan.
• Provide ADL support (bathing, grooming, toileting, meals).
• Safely transport individuals in company or personal vehicles.
• Accompany individuals to community activities.
• Complete documentation in the EHR timely.
• Engage with individuals in home and community.
• Uphold rights and HIPAA confidentiality.
• Maintain professional boundaries and ethics.
• DSPs are mandated reporters for abuse/neglect/exploitation.
• Administer prescribed medications after training.
• Be proficient with technology and stay awake during shifts.
• Perform physical tasks including lifting and transfers.
• Other duties as assigned.
