Job summary

UnitedHealth Group is seeking a HouseCalls Advanced Practice Clinician (NP) to perform in-home health assessments for Medicare Advantage and other plans. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes.

The ideal candidate is an NP with ANCC or AANP certification, an unrestricted license, and the ability to navigate diverse home environments. A flexible schedule and meaningful patient impact are offered.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active and unrestricted license in the practice state (or able to obtain by start date).
• Active and unrestricted driver's license with reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Conduct comprehensive in-home assessments including medical history review, vitals, and exams.
• Document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address care gaps.
• Identify urgent/emergent situations and intervene appropriately.
• Educate members on disease processes, medications and adherence.
• Address social determinants of health and provide referrals as needed.
