Qualifications

• Active licensure in practicing state without disciplinary actions.
• Certification as NP or PA per role (Family/Adult/Geriatric for NP; NCCPA for PA).
• Excellent communication and cultural sensitivity for diverse patients.

Responsibilities

• Conduct in-home assessments for Medicare Advantage and other plans.
• Document diagnoses for care management and plan treatment.
• Educate patients on disease processes, medications, and adherence.
• Coordinate with PCPs to address gaps in care and referrals.
• Respond to urgent situations during home visits.
