UnitedHealth Group is seeking a HouseCalls Advanced Practice Clinician (Nurse Practitioner) to perform in-home health assessments for Medicare Advantage members. This field-based role emphasizes care coordination, patient education, and collaboration with PCPs to close care gaps.

Requirements include active ANCC/AANP certification, unrestricted state licensure, and a valid driver's license with reliable transportation for home visits.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Unrestricted licensure in the practicing state (or ability to obtain by start date)
• Active and unrestricted driver’s license, reliable transportation for home visits
• Ability to navigate varied home environments and perform assessments
• If state allows NP/PA independence, obtain licensing board approval within specified timelines

Responsabilités

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members
• Document diagnoses for care management and treatment planning
• Communicate findings to primary care providers to address gaps in care
• Identify urgent situations and intervene appropriately
• Educate members on disease processes, medications, and adherence
• Address social determinants of health and provide referrals as needed
