You will evaluate, treat, and document progress using Cliniconnects EMR, coordinate with care teams, and tailor treatment plans to individual needs while maintaining compliance and high standards of care.

Qualifications

• Licensed PT in CA with active license.
• Experience delivering home health PT across age groups preferred.
• Proficient in documentation and patient progress reporting.

Responsibilities

• Deliver home health PT to patients in Lemon Grove area.
• Assess, plan, and document patient treatment plans.
• Coordinate with care teams and adjust plans as needed.
• Maintain patient records in EMR with accurate progress notes.
