Job summary

FeldCare Connects, an app-based referral network, seeks a compassionate Physical Therapist to deliver quality home healthcare to patients of all ages in Perris and surrounding areas. This independent contractor role offers control over your schedule and coverage areas.

Work autonomously with administrative support and access to an intuitive EMR for streamlined documentation. You determine your earnings through per-visit rates and patient load while prioritizing patient outcomes.

Qualifications

• Ability and enthusiasm in working with all ages.
• Current Physical Therapist license issued by the state board; license must be current, active, and unrestricted; conditional/provisional/restricted licenses not accepted.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Smartphone.
• Bilingual a plus!

Responsibilities

• Provide one-on-one home-based physical therapy to patients in Perris and surrounding areas.
• Observe, record, and report patient responses to treatments; determine and adjust plan of care.
• Complete documentation efficiently using the EMR system during visits.
