Job summary

FeldCare Connects seeks an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages. This Independent Contractor role serves Stockton and surrounding areas, offering flexible scheduling and the ability to control patient mix through per-visit compensation.

You will operate as your own boss with administrative support from FeldCare Connects, using Cliniconnects to manage patient referrals and acceptance in seconds.

Qualifications

• Current Physical Therapist license issued by the state Board of Physical Therapy.
• License must be active, unrestricted; conditional or restricted licenses will not be accepted.
• Completion of an accredited Physical Therapy program.

Responsibilities

• Observe, record, and report patient responses to treatment.
• Determine and implement plan of care.
• Provide one-on-one treatment and home care.
• Complete documentation using the EMR system during visits.
