Job summary

FeldCare Connects is seeking an enthusiastic Physical Therapist to deliver quality home healthcare in Tracy and surrounding areas as an independent contractor. You will provide one-on-one treatment to patients of all ages, using our Cliniconnects app for referrals and scheduling, and document progress in the EMR during each visit.

Qualifications include an active PT license in California, completion of an accredited PT program, reliable transportation, and a smartphone.

Qualifications

• Must be currently licensed as a Physical Therapist in the state with an active, unrestricted license.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Smartphone access.
• Bilingual ability is a plus.

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report patient responses to treatment and adjust the plan of care as needed.
• Provide one-on-one treatment and determine the plan of care.
• Use Cliniconnects app to accept/decline patients and coordinate scheduling.
• Document patient visits and progress in the EMR during or immediately after sessions.
