Job summary

Enlyte is seeking a Field Case Manager in Oceanside, CA, a full-time, hybrid position requiring regular local travel for in-person patient appointments.

You will collaborate with physicians, employers, and patients to create effective treatment plans, advocate for patients, and ensure cost-efficient healthcare services and timely recovery to work. Strong nursing background and case management certification are preferred.

Qualifications

• Associates or Bachelor's degree in Nursing or related field.
• 2+ years of clinical practice preferred.
• Active RN license in good standing.
• URAC-recognized case management certification (CCM, CDMS, CRC, CRRN or COHN...).

Responsibilities

• Create and implement treatment plans for injured/disabled individuals.
• Coordinate with physicians/providers, employers, and customers.
• Assist in modifying job duties to facilitate return to work.
• Attend in-person appointments as needed and provide status updates.
