Enlyte is seeking a Field Case Manager in Riverside, CA for a full-time hybrid role requiring local travel for in-person appointments. You will collaborate with physicians, employers, and clients to plan and monitor treatment, ensuring safe, cost-effective care and timely return to work.

Qualifications

• Active RN license in good standing.
• 2+ years of clinical practice preferred.
• URAC-recognized case management certifications (CCM, CDMS, CRC, CRRN, COHN/COHN-S, RN-BC, ACM, CMAC, CMC).
• Must have reliable transportation for local travel to appointments.

Responsibilities

• Create and implement treatment plans with physicians/providers and employers.
• Advise on appropriate and cost-effective healthcare services.
• Return injured workers to work safely and promptly.
• Manage workload, prioritize cases, and update customers with status.
• Attend in-person appointments when needed and coordinate care.
