The position emphasizes building a long-term relationship, flexible daytime hours, and distance travel to meet client needs. Ideal candidates bring 1+ year home health or case management experience, nursing supervision capabilities, and Ohio RN licensure.

Qualifications

• Minimum 1 year of experience in case management, admissions and discharges.
• 1+ year paid experience at a home health agency or in-home care.
• Nursing supervisory experience preferred.
• Graduate of an accredited School of Nursing with Ohio RN licensure.

Responsibilities

• Perform client assessments and develop care plans.
• Supervise a team of RNs, LPNs, HHAs, and PCAs.
• Coordinate communication between physicians, staff and families.
• Travel to surrounding counties as needed.
