Job summary

CVS Health is seeking a Registered Nurse Case Manager – Field in New Jersey to coordinate care for members with complex needs in a community-based setting. This role emphasizes independent work, compassionate care, and partnership with families and providers.

You will conduct in-home assessments, tailor care plans, and mentor teammates while navigating the EHR and care transitions to improve safety and quality of life.

Qualifications

• RN license in New Jersey required.
• Minimum 3 years of clinical experience in hospital, home health, or ambulatory care.
• NJ Choice Certification with minimum exam score of 80% and completion of state training modules.

Responsibilities

• Perform in-home assessments for members enrolled in MLTSS programs.
• Evaluate medical, behavioral, and social needs to plan appropriate care.
• Collaborate with providers, caregivers, and interdisciplinary teams for coordinated care.
• Develop and manage individualized care plans to promote safety and independence.
• Document assessments and care activities in the EHR.
• Support care transitions and connect members with community resources.
• Mentor new team members as you grow in the role.
